The Online Booking service now opens from 08:15 in the morning Monday - Friday.


All our Patients have a named GP who is responsible for their overall care at the practice. We listen when Patients make us aware of their preference as to which GP that is. If you are registering as a new patient you will see an option to choose, you will also receive a text message giving you the GP’s name, alternatively you can contact the practice if you wish to know who this is.


 

Registration with the Practice

PreRegistrationIf you wish to register and you live in the practice boundary please complete the form below on a PC (some mobiles/tablets might work but we know there are problems). This is now in 'Word' format so you can save it and finish or submit it later. University of Leicester students (there is no such arrangements for staff, sorry) should contact our reception staff if they live outside the above boundary as they may still be able to register.

IMPORTANT - USUAL ERRORS
INCOMPLETE FORMS - Please check every question has been answered.
Please give your NHS number if you have one. It is in this format nnn nnn nnnn, for example 123 456 7890.
You MUST expect to live in the practice boundary for 3 months or more (see 'Temporary Resident' if you are here for less than 3 months).
If you have NEVER had a UK GP before, we need the date you entered the UK
The Previous address for all applicants is the one your previous UK GP had for you.
If you get a message to call us about your registration it is usually because the NHS has a different address than the one you gave us, please call us urgently.

HOW TO SUBMIT
If you have access to a Fax machine or Fax app on your smartphone or tablet/laptop/PC you can send it to us on 0116 2151100
You can also print it off after completion and bring it in or post it to us at the above address
Or you can email it to us. Please be aware that sending from an account that is not an nhs.net one is not guaranteed to be safe due to security risks, if you choose to take the risk please use fchc@nhs.net (THIS EMAIL IS NOT TO BE USED BY COLD CALLERS, SUPPLIERS OR FOR ANY OTHER PURPOSE AND WE WILL NOT USE THIS ACCOUNT TO REPLY).

WHAT HAPPENS NEXT
If you have included a mobile number and consented to receiving messages we will send you a message when your application has been processed. You should then bring in identification to obtain your ONLINE SERVICES access details.
